Abstract Kronična upalna bolest crijeva (eng. IBD- inflammatory bowel disease) je autoimuna sistemska bolest, koja uz interakciju genetike i okoliša, uzrokuju upalu, uglavnom lokaliziranu u sluznici probavnog trakta. Prema podjeli odnosi se na Crohnovu bolest i ulcerozni kolitis. Iako su te dvije bolesti opisane odvojeno, uvijek ih se promatra zajedno, jer ih označavaju rekurentne upale pojedinih segmenata probavnog sustava s vrlo raznolikim kliničkim očitovanjima i u pravilu kroničnim, ali i nepredvidivim tijekom bolesti. U 20-25% slučajeva simptomi tih dviju bolesti počinju u ranom djetinjstvu ili adolescenciji. Osim gastrointestinalnim simptoma (kao što su dijarea i bolovi u trbuhu), za pedijatrijsku dob su karakteristične ekstraintestinalne manifestacije bolesti vezane za pojavu bolesti prije pubertetskog zamaha rasta. Tako loša regulacija bolesti uz slabije cijeljenje sluznice i povećane koncentracije citokina, malnutricija, povećane potrebe za energijom i kortikosteroidi u terapiji dovode do deficita brojnih tvari u organizmu, zastoja u rastu, zakašnjelog puberteta, te smanjenje gustoće kostiju. Glavni ciljevi terapije su poboljšati rast i razvoj, smanjujući upalu sa što dužim razdobljima remisije uz poštedu od terapije kortikosteroidima, te održati adekvatnu uhranjenost, kako bismo postigli očekivanu konačnu visinu, povećali kvalitetu života pacijenata, uz održan normalan fizički, spolni, intelektualni i psihosocijalni razvoj.
U ovom radu detaljno sam opisala tijek i nastanak Crohnove bolesti i ulceroznog kolitisa. Nadalje, opisala sam epidemiološke podatke, kliničku sliku, simptome i komplikacije prije navedenih upalnih bolesti crijeva. Opisala sam dijagnostiku i liječenje bolesnika uz preporuke na specifičnu prehranu oboljelih. Također sam se osvrnula na ulogu medicinske sestre u pružanju zdravstvene njege, kao i edukacije roditelja i djece oboljelih od kroničnih upalnih bolesti crijeva.
Abstract (english) Chronic inflammatory bowel disease is an autoimmune systemic disease that, with the interaction of genetics and the environment, causes inflammation, mainly localized in the digestive tract mucosa. The division refers to Crohn's disease and ulcerative colitis. Although these two diseases are described separately, they are always monitored together because they indicate recurrent inflammation of the individual digestive tract segments with very diverse clinical manifestations and, in general, chronic but unpredictable during the illness. In 20-25% of cases the symptoms of these two diseases begin in early childhood or adolescence. Osim gastrointestinalnim simptoma (dijarea i bolovi u trbuhu) za pedijatrijsku dob su karakteristične ekstraintestinalne manifestacije bolesti vezane za pojavu bolesti prije pubertetskog zamaha rasta. Poor regulation of the disease with severe mucus healing and increased cytokine, malnutrition, increased energy needs, and corticosteroids in therapy lead to deficiencies of many nutrients in the body, growth failure, delayed puberty, and bone density reduction. The main goals of the therapy are to improve growth and development, reducing inflammation with as long as possible remission periods in addition to corticosteroid therapy, and maintain adequate nutrition to achieve the expected ultimate height, increase the quality of life of patients, while maintaining normal physical, sexual, intellectual and psychosocial development.
In this thesis, I have described in detail the course and the occurrence of Crohn's disease and ulcerative colitis. Furthermore, I have described epidemiological data, clinical picture, symptoms and complications of previously mentioned inflammatory bowel diseases. I have described diagnosis and treatment of patients with recommendations for specific nutrition for the patients. I also looked at the role of nurse in providing health care as well as education of parents and children affected by chronic inflammatory bowel disease.
