Abstract Kronične rane danas predstavljaju velik problem, kako bolesnicima tako i medicinskom osoblju, ali u konačnici i cijelom zdravstvenom sustavu. Sve one rane koje ne zacijele u razdoblju od 6 do 8 tjedna nazivamo kroničnim ranama. Možemo ih podijeliti na tipične i atipične rane. U većini slučajeva pojavljuju se venski ulkus, arterijski ulkus, dijabetičko stopalo i dekubitus. Bolesniku s kroničnom ranom potrebna je pomoć u obavljanju svakodnevnih životnih aktivnosti te vrlo često iziskuje uključivanje obitelji.
Troškovi liječenja kroničnih rana nisu zanemarivi u odnosu na neke druge bolesti i vrlo je važno na koji način i kojim materijalima će se rane tretirati. Etiološki čimbenici kroničnih rana zahtjevaju multidisciplinaran pristup uz velik trud zdravstvenog osoblja s ciljem da se kronična rana tretira što jednostavnije, produktivnije i bezbolnije za bolesnike, a ujedno što jeftinije za zdravstveni sustav. Kod kroničnih rana liječenje treba usmjeriti u cjelini prema bolesniku, a ne se samo bazirati na liječenje rane. Kronične rane mogu se vrlo učinkovito i stručno tretirati i u kući bolesnika. Zdravstvenu njegu u kući bolesnika provode Ustanove za zdravstvenu njegu u kući. Zdravstvena njega u kući predstavlja neprekinuti lanac pružanja skrbi bolesniku nakon otpusta iz bolnice ako to njegovo stanje zahtjeva. Zdravstvena njega u kući djelatnost je koja se provodi na razini primarne zdravstvene zaštite. Temelj dobrog provođenja zdravstvene njege u kući ovisi o sestrinskoj procjeni te o pravodobnom i kvalitetnom djelovanju u okviru vještina i timskog rada. Sama procjena kroničnih rana zahtjeva dodatna znanja i iskustva medicinske sestre, kao i različite prilagodbe i napore. Medicinska sestra u kući kod pacijenta s kroničnom ranom najčešće nailazi na neke od problema, a to su: bol, oštećenje integriteta kože, neupućenost, strah, visok rizik za pad i infekciju te smanjenu mogućnost brige o sebi. Tijekom pružanja zdravstvene njege u kući bolesnika medicinska sestra uspostavlja komunikaciju između bolesnika i njegove obitelji te mu prilagođava njegu s drugim svakodnevnim životnim aktivnostima. Sve medicinske sestre/ tehničari koji rade u zdravstvenoj njezi u kući imaju jednaki cilj, a to je poticanje bolesnika na što veću samostalnost u obavljanju i zadovoljavanju osnovnih ljudskih potreba kako bi unaprijedili i očuvali kvalitetu života bolesnika.
Abstract (english) Chronic wounds today present a major problem for both patients and medical personnel, but ultimately the entire health care system. All those wounds that do not heal within 6 to 8 weeks are called chronic wounds. We can divide them into typical wounds and atypical ones. In most cases, venous, arterial ulcer, diabetic foot and decubitus occur. Therefore, such a patient needs help in performing daily life activities and requires the involvement of his family.
The costs of treating chronic wounds are not negligible compared to some other diseases and it is very important how and in what materials the wounds will be treated. The etiological factors of chronic wound require a multidisciplinary approach with a great effort by health care personnel with the aim of treating the chronic wound as simple, productive and painless as possible for patients, while cheaply as possible for the health care system. For chronic wounds, treatment should be directed as a whole towards the patient and not based solely on wound treatment. Chronic wounds can be treated in the patient's home very efficiently and professionaly. Patient's home health care is performed by institutions for home health care. Health care in the house is a continuous chain of care for a patient after discharge from hospital if his condition requires. House health care is an activity implemented at primary health care level. The foundation for good home health care depends on the sorority assessment, timely and quality work within skills and team work. The mere assessment of chronic wounds requires additional knowledge and expirience from the nurse, as well as various adjustments, additional expertise and efforts. The nurse in the house in a patient with chronic wounds most often encounters some of the problems, namely: pain, damage to skin integrity, ignorance, fear, high risk of falling and infection, and reduced ability to take care of themselves. During health care in the patient's home, the nurse establishes communication between the patient and his/her family and adapts care to other dailiy life activities. All nurses/technicians working in the house have the same goal, which is to encourage patients to be as independet as possible in performing and meeting basic human needs in order to improve and preserve the quality of life of patients.
