Abstract
Prijelomi zdjelice mogu nastati kao posljedica niskoenergetske ili visokoenergetske traume. Prijelomi zdjelice mogu biti povezani sa značajnim unutarnjim krvarenjem i ozljedom organa unutar zdjelice. Pacijenti obično imaju složene zdravstvene potrebe. Prijelomi zdjelice čine 3% skeletnih ozljeda, najčešće je uzrok visoka energija kod nesreća motociklista ili pješaka. Dva najčešće korištena klasifikacijska sustava su klasifikacija Tile i klasifikacija Young-Burgess. Klasifikacija Tile uzima u obzir stabilnost zdjelice dok se Young-Burgessova klasifikacija temelji se na mehanizmu ozljede. Zbrinjavanje pacijenta može se podijeliti na akutnu fazu i fazu definitivnog zbrinjavanja. Zbrinjavanje pacijenta uključuje temeljitu procjenu ozljeđenika, temeljitu procjena hemodinamskog statusa, stanja kralješnice i dišnih puteva te prisutnost drugih ozljeda i prijeloma. U dijagnostici se najčešće koristi kompjuterizirana tomografija (CT), iako je najbolji test za probir prijeloma zdjelice anteroposteriorna rendgenska snimka (RTG snimka) zdjelice. Daljnje liječenje uključuje stabilizaciju zdjelice,a definitivni tretman kada je pacijentovo stanje stabilno. Medicinske sestre/tehničari imaju središnju funkciju u praćenju pacijenata nakon ovakvih ozljeda i stoga su u izvrsnoj poziciji za prepoznavanje potencijalno životno opasnih situacija ili pogoršanja stanja pacijenta. Značajne ozljede zdjelice zahtijevaju dulje razdoblje imobilizacije i mirovanja pacijenta u krevetu. Aktivnosti medicinske sestre/tehničara kod konzervativnog načina liječenja usmjerene su na prevenciju komplikacija dugotrajnog mirovanja, a kod operacijskog liječenja na pružanje adekvatne perioperacijske zdravstvene skrbi. Zdravstvena njega pacijenta s prijelomom zdjelice je izuzetno složena jer često uključuje opsežan i kompliciran operacijski zahvat zbog kojeg pacijent ima poteškoće s hranjenjem, uređivanjem te odlaskom na toalet. Takav operacijski zahvat uvelike može pridonijeti stresu i strahu pacijenta, ali i komplikacijama, kao što su dekubitusi i/ili infekcije, koji pridonose produljenom razdoblju imobilizacije.
Abstract (english)
Pelvic fractures can occur as a result of low-energy or high-energy trauma. Pelvic fractures may be associated with significant internal bleeding and organ injury within the pelvis. Patients usually have complex health needs. Pelvic fractures account for 3% of skeletal injuries, most often causing high energy in motorcyclist or pedestrian accidents. The two most commonly used classification systems are the Tile classification and the Young-Burgess classification. The Tile classification is based on pelvic stability while the Young-Burgess classification is based on the injury mechanism. Patient care can be divided into the acute phase and the definitive care phase. Patient care includes a thorough assessment of the injured party, a thorough assessment of hemodinamic status, vertebral and respiratory conditions, and the presence of other injuries and fractures. Computed tomography is most commonly used in the diagnosis, although the best test for pelvic fracture screening is an anteroposterior X-ray of pelvis. Further treatment includes pelvic stabilization, and definitive treatment when the patients condition is stable. Nurses/technicians have a central function in monitoring patients after such injuries and are therefore in an excellent position to identify potentially life-threatening situations or exacerbate the patient's condition. Significant pelvic injuries require a longer period of immobilization and rest of the patient in bed. The activities of a nurse / technician in a conservative treatment are aimed at preventing complications of long-term rest, and in surgical treatment at the provision of adequate perioperative health care. The health care of a patient with a pelvic fracture is extremely complex because it often involves extensive and complicated surgery that causes the patient to have difficulty feeding, getting dressed and going to the toilet. Such surgery can greatly contribute to the stress and fear of the patient, but also to complications, such as bedsores and / or infections, that contribute to an extended period of immobilization.
