Abstract Politrauma se definira kao ozljeda koja zahvaća najmanje dva sustava ili dva različita organa, od kojih jedna ozljeda je opasna po život pacijenta. Stoga je politrauma jedan od najvećih javnozdravstvenih problema današnjice. Prema statistici, politraumu čini 3-8% svih ozljeda, a smrtnost pacijenta s politraumom je 25-35%. Zbrinjavanje politraume se može odvijati na nekoliko razina. Na samom mjestu nesreće započinje zbrinjavanje politraumatiziranog pacijenta, te se nastavlja tijekom transporta, a dovršava u bolničkoj ustanovi. Zbrinjavanje pacijenta s višestrukim traumama odnosno politraumama vrlo je složen proces jer zahtijeva brzo i učinkovito timsko djelovanje. Osnovna načela rada izvanbolničke hitne medicinske pomoći su spašavanje naglo ugroženog ljudskog života kroz osnovne i napredne metode održavanja života. Svako zbrinjavanje pacijenata s politraumom regulirana je nacionalno priznatim smjernicama pod nazivom International Trauma Life Support (ITLS). Prema ITLS smjernicama, procjena stanja pacijenta započinje primarnim pregledom, zatim sekundarnim pregledom, a završava kontrolnim pregledom. Prilikom zbrinjavanja politraumatiziranog pacijenta osnovni je cilj je pružiti adekvatnu prvu pomoć, te napraviti brzu procjenu fizikalnim pregledom, a pritom omogućiti što bolju oksigenaciju organizma, zaustaviti vanjsko krvarenje i što je prije moguće započeti s nadoknadom izgubljenog volumena, čime se smanjuje mogućnost nastanka hipovolemijskog šoka i oštećenja tkiva. Zbrinjavanje politraumatiziranih pacijenata je jedno od najkompleksnijih stanja, pa zbrinjavanje takvih pacijenata zahtjeva visoko obrazovanje izvanbolničkog tima, kontinuiranu edukaciju, te razvijene specijalne vještine kako bi se u pojedinim situacijama reagiralo brzo i na kvalitetan način.
Cilj ovog rada je prikazati najčešće politraume koje se zbrinjavanju u izvanbolničkoj hitnoj službi, načini procjene pacijenta i postupci prilikom zbrinjavanja i liječenja životno ugroženog pacijenta.
Abstract (english) Polytrauma is defined as an injury involving at least two systems or two different organs, one of which is life-threatening. Therefore, polytrauma is one of the biggest public health problems of today. According to statistics, polytrauma accounts for 3-8% of all injuries, and the mortality of a patient with polytrauma is 25-35%. Care for polytrauma can take place on several levels. At the scene of the accident, the care of a polytraumatized patient begins, and continues during transport, and ends at the hospital. Caring for a patient with multiple traumas or polytraumas is a very complex process because it requires fast and efficient teamwork. The basic principles of outpatient emergency medical care are the rescue of suddenly endangered human life through basic and advanced methods of life support. Any care for patients with polytrauma is regulated by nationally recognized guidelines called International Trauma Life Support (ITLS). According to ITLS guidelines, the assessment of a patient's condition begins with a primary examination, then a secondary examination, and ends with a follow-up examination. When caring for a polytreumatized patient, the main goal is to provide adequate first aid, and make a quick assessment by physical examination, while enabling better oxygenation of the body, stop external bleeding and start as soon as possible to compensate for lost volume, thus reducing the possibility of hypovolemic shock and tissue damage. Caring for polytraumatized patients is one of the most complex conditions, and therefore caring for such patients requires a highly educated outpatient team, continuous education, and developed special skills to respond to certain situations quickly and in a quality manner.
The aim of this paper is to present the most common polytraumas that are treated in outpatient emergency services, methods of patient assessment and procedures in the care and treatment of life-threatening patients.
