Abstract Prema dobivenim podacima iz HZJZ-a, možemo reći kako je incidencija karcinoma mokraćnog mjehura u porastu. Na petom je mjestu kod muškaraca, dok je na skoro zadnjem mjestu kod žena. Radikalno liječenje karcinoma mokraćnog mjehura zahtijeva multidisciplinarni i holistički pristup pacijentima. Vrednovanje svakog segmenta procesa rada u liječenju i zbrinjavanju daje zadovoljavajući rezultat konačnog liječenja, stoga je važno pomno promatrati svakog pojedinca u njegovom radu kako bi se izbjegle nepoželjne situacije, kako za pacijenta tako i za kolektiv. U liječenju pacijenata s karcinomom mokraćnog mjehura često se mogu povezati i infekcije mokraćnog sustava. Pravovremeno zbrinjavanje takvog pacijenta može dati adekvatan odgovor u samom procesu radikalnog liječenja. Iako je bolnička okolina podloga za dodatni razvoj infekcija, valja se prisjetiti da smo upravo mi, zdravstveni radnici oni koji mogu pridonijeti većoj i boljoj kvaliteti boravka i samog oporavka pacijenata, jer infekcija znači duži boravak, a time i skuplji.
Prikupljeni su podatci za istraživanje u razdoblju od 1.1.2015. godine do 31.12.2020. godine u Kliničkoj bolnici „Sv. Duh“, Zavod za urologiju. Uzorak čine 264 pacijenta s područja Republike Hrvatske. Istraživanje je ustrojeno kao retrospektivna studija uz anketu koju su ispunjavale 22 medicinske sestre, odnosno tehničari. Prikupljeni podaci obrađeni su računalnim programom Microsoft Excel.
Primarni je cilj bio istražiti povezanost između infekcija nastalih nakon operacije sa znanjem medicinskih sestara ili tehničara, tj. detektiranje pozitivnih urinokultura prije i nakon radikalnog zahvata, te povezivanje s anketom koja je prevedena među zaposlenicima. Najčešći detektirani izolat je Candida, koja je rezultat profilakse antibiotika i novog neprirodnog načina mokrenja kroz stomu. Rezultati su pokazali kako se osoblje snalazi u zahtjevnim situacijama, kao što su provođenje dijagnostike i obavljanje zdravstvene njege kod bolesnika s određenom mjerom kontaktne izolacije.
Učestalim ponavljanjem znanja o intrahospitalnim infekcijama u sustavu zdravstva i provođenjem istinske kontrole, uvelike će se smanjiti troškovi zdravstvenih ustanova, dok su korist pacijenta i njegov daljnji tijek oporavka nemjerljivi.
Abstract (english) According to the data obtained from the HZJZ, we can say that the incidence of bladder cancer is increasing. It is in the fifth place among men, while it is almost the last on the list among women. Radical treatment of bladder cancer requires a multidisciplinary and holistic approach to patients. Evaluating each segment of the work process in patient treatment and care leads to a satisfactory result in the final treatment, therefore it is important to closely observe each individual in his work in order to avoid undesirable situations for both the patient and the collective. Urinary tract infection can often be associated with the treatment of patients with bladder cancer. Timely treatment of such a patient can provide an adequate response in the very process of radical treatment. Although the hospital environment is the basis for the additional development of infections, it should be remembered that it is us, the healthcare workers, who can contribute to a greater and better quality of the patients' stay and recovery, because infection means longer and also more expensive stay.
A five-year study was conducted in the period from January 1, 2015 until December 31, 2020 in the "St. Duh" Clinical Hospital, Institute of Urology. The sample consists of 264 patients from the Republic of Croatia. The research was organized as a retrospective study with a survey completed by 22 nurses and technicians. The collected data were processed in the Microsoft Excel computer program.
The primary goal was to investigate the connection of infections occurring after surgery with the knowledge of nurses or technicians, i.e. the detection of positive urine cultures before and after a radical procedure, and the connection with the survey conducted among the employees. The most common isolate that was detected was Candida, which is the result of antibiotic prophylaxis and the new unnatural way of urinating through the stoma. The results showed how the staff copes with demanding situations, such as conducting diagnostics and providing health care to patients with a certain degree of contact isolation.
Frequent renewal of knowledge about intrahospital infections in the healthcare system and the implementation of true control will greatly reduce the costs of healthcare institutions, while the benefit of the patient and his further course of recovery are immeasurable.
