Abstract Gestacijski dijabetes po prvi puta se dijagnosticira u trudnoći, a karakteriziran je pojavom inzulinske rezistencije. Točan uzrok njegovog nastanka još nije u potpunosti istražen, ali se smatra da na njegov razvoj utječu hormoni do čijeg lučenja dolazi za vrijeme trudnoće. Veći rizik za razvoj gestacijskog dijabetesa imaju trudnice koje imaju prekomjernu tjelesnu masu, koje su starije životne dobi te koje za vrijeme trudnoće prekomjerno dobivaju na tjelesnoj težini. Obično se javlja u drugom tromjesečju trudnoće, a konačna dijagnoza gestacijskog dijabetesa postavlja se na temelju oralnog glukoznog tolerans testa. Gestacijski dijabetes najčešće se liječi uvođenjem raznovrsne i zdrave prehrane koja mora imati određeni broj kalorija kako bi one trudnici osigurale dovoljno energije. Osim pravilne prehrane, trudnicama se preporuča vježbanje koje zajedno sa pravilnom prehranom pokazuje učinkovite rezultate i dovodi do normalizacije razine glukoze u krvi. Ukoliko se navedenim metodama ne omogući zadovoljavajući učinak na razinu glukoze u krvi, uvodi se farmakološko liječenje koje podrazumijeva aplikaciju inzulina. Gestacijski dijabetes utječe i na trudnicu i na dijete. Utječe na intrauterini i postnatalni razvoj ploda te često dovodi do rađanja djeteta s prekomjernom porođajnom težinom, odnosno makrosomijom. Kako bi se na vrijeme spriječile te komplikacije, potrebno je prije svega procijeniti opći i funkcionalni status trudnice te nakon toga izraditi plan fizioterapije koji će imati pozitivan utjecaj na ishod trudnoće. Procjena obuhvaća procjenu posture, procjenu tjelesnih aktivnosti kod trudnice te procjenu utjecaja gestacijskog dijabetesa na trudnicu. Procjena se provodi putem raznih upitnika i testova koji daju povratnu informaciju o ispitivanim funkcijama i omogućuju da se trudnici pristupi individualno te se na temelju njih izrađuje plan fizioterapijskih intervencija. Fizioterapijske intervencije bazirane su na vježbama jačanja svih mišićnih skupina, vježbama za cirkulaciju i vježbama disanja. Vježbe trebaju biti prilagođene mogućnostima trudnice, trebaju biti umjerenog intenziteta i u skladu s mogućnostima trudnice. Tijekom vježbanja, uloga fizioterapeuta je da nadzire kako trudnica izvodi vježbe te da ju korigira ukoliko je potrebno. Isto tako je bitno da se za vrijeme vježbanja obrati pozornost na eventualne znakove i simptome koji se mogu javiti i koji ukazuju na prekid vježbanja.
Abstract (english) Gestational diabetes is diagnosed for the first time during pregnancy, and is characterized by the appearance of insulin resistance. The exact cause of its occurrence has not yet been fully investigated, but it is believed that its development is influenced by hormones secreted during pregnancy. Pregnant women who are overweight, who are older and who gain excessive weight during pregnancy have a greater risk of developing gestational diabetes. It usually occurs in the second trimester of pregnancy, and the final diagnosis of gestational diabetes is made on the basis of an oral glucose tolerance test. Gestational diabetes is most often treated by introducing a varied and healthy diet that must have a certain number of calories in order to provide the pregnant woman with enough energy. In addition to proper nutrition, pregnant women are recommended to exercise, which, together with proper nutrition, shows effective results and leads to the normalization of blood glucose levels. If the mentioned methods do not provide a satisfactory effect on the level of glucose in the blood, pharmacological treatment is introduced, which includes the application of insulin. Gestational diabetes affects both the pregnant woman and the child. It affects the intrauterine and postnatal development of the fetus and often leads to the birth of a child with excessive birth weight, i.e. macrosomia. In order to prevent these complications in time, it is first of all necessary to assess the general and functional status of the pregnant woman and then create a physiotherapy plan that will have a positive impact on the outcome of the pregnancy. The assessment includes assessment of posture, assessment of physical activities in pregnant women and assessment of the impact of gestational diabetes on pregnant women. The assessment is carried out through various questionnaires and tests that provide feedback on the tested functions and allow the pregnant woman to be approached individually, and based on them, a plan of physiotherapy interventions is drawn up. Physiotherapy interventions are based on exercises to strengthen all muscle groups on the body, followed by exercise for circulation and breathing exercises Exercises should be adapted to the possibilities of the pregnant woman, they should be of moderate intensity and in accordance with the possibilities of the pregnant woman. During exercise, the role of the physiotherapist is to monitor how the pregnant woman performs the exercises and to correct her if necessary. It is also important to pay attention to possible signs and symptoms that may occur during exercise and that indicate the cessation of exercise.
