Abstract Srčano-žilne bolesti danas su vodeći uzrok mortaliteta i obolijevanja u svijetu, te predstavljaju velik javnozdravstveni problem. Infarkt miokarda posljedica je koronarne tromboze i posljedične ishemije koja traje dulje vrijeme te je dostatno intenzivna da bi izazvala nekrozu miokarda. Postoje mnogi čimbenici rizika koji se povezuju s nastankom aterosklerotskih promjena i srčano-žilnih bolesti, te međusobno djeluju godinama. Najvažniji biomarker za dijagnostiku akutnog infarkta miokarda je porast vrijednosti troponina koji označuje stanje odnosno raspadanje srčanog mišićnog tkiva. Danas je vrlo bitna uloga fizioterapije za unaprjeđenje i prevencije nastanka bolesti kao i poticanje vođenja zdravog načina života. Fizioterapijom se provode razni postupci, od procjene, intervencije te u konačnici evaluacije rezultata. Procjenom je potrebno utvrditi mišićnu snagu, motiviranost pacijenta, razinu aerobnog kapaciteta i psihičko stanje pacijenta. Široko priznata činjenica jest da fizička aktivnost doprinosi dobrobiti za kardiovaskularnu funkciju, smanjuje faktore rizika i mortalitet bez obzira na dob. Kardiološka rehabilitacija definirana je kao skup aktivnosti odnosno postupaka koji su bitni da se kardiološkom pacijentu osiguraju najbolji fizički, psihički i socijalni uvjeti da bi samostalno mogao sačuvati ili ponovno ostvariti što normalnije funkcioniranje u vlastitom životu te u okruženju svoje obitelji i zajednice. Pri provođenju kardiološke rehabilitacije potrebna je timska suradnja zdravstvenih djelatnika u specijaliziranim bolničkim ili ambulantnim centrima. Ciljevi kardiološke rehabilitacije obuhvaćaju smanjenje progresije bolesti, poboljšanje mentalnog zdravlja, prevencija novih kardiovaskularnih incidenata, te postizanje maksimalnog zdravlja u skladu s mogućnosti. Prva faza rehabilitacije započinje u jedinici koronarne ili kardiokirurške intenzivne skrbi, uz provođenje rane mobilizacije pacijenta za sprječavanje komplikacija dugotrajnog ležanja. Druga faza rehabilitacije provodi se ambulantno u zdravstvenim ustanovama, intrahospitalno ili rjeđe u kućnim uvjetima. Provodi se aerobni trening, vježbe s otporom, vježbe disanja te ako postoje mogućnosti i vježbe u bazenu. Treća faza rehabilitacije označuje dugotrajno to jest doživotno održavanje pacijenata. Dodatno se motivira pacijente da poboljšaju životne navike, prestanak pušenja, zdravija prehrana, te izvođenje redovite tjelesne aktivnosti.
Abstract (english) Today cardiovascular diseases are the leading cause of mortality and morbidity in the world, and they represent a major public health problem. Myocardial infarction is the result of coronary thrombosis and consequent ischemia that lasts for a long time and is intense enough to cause myocardial necrosis. Many risk factors are associated with the development of atherosclerotic changes and cardiovascular diseases which interact for years. The most important biomarker for the diagnosis of acute myocardial infarction is an increase in the troponin value, which indicates the state or breakdown of heart muscle tissue. Today, the role of physiotherapy is especially important for the improvement and prevention of the onset of diseases, as well as the encouragement of leading a healthy lifestyle. Various procedures are carried out in physiotherapy, from assessment, intervention and finally evaluation of the results. The assessment must determine the patient's muscle strength, motivation, level of aerobic capacity and the patient's psychological state. It is a widely recognized fact that physical activity contributes to the well-being of cardiovascular function, reduces risk factors and mortality regardless of age. Cardiac rehabilitation is defined as a set of activities or procedures that are essential to provide the cardiology patitent with the best physical, psychological and social conditions so he can independently preserve or regain normal functioning in his own life and in the environment of his family and community. When carrying out cardiac rehabilitation, team cooperation of health professionals in specialized hospital or outpatient centers is necessary. The goals of cardiac rehabilitation include reducing disease progression, improving mental health, preventing new cardiovascular incidents, and achieving maximum health as possible. The first phase of rehabilitation begins in the coronary or cardiosurgical intensive care unit with early mobilization of the patient to prevent complications of prolonged lying down. The second phase of rehabilitation is carried out on an outpatient basis in health institutions, intra-hospital or less often at patient's home. Aerobic training, resistance exercises, breathing exercises and if there are possibilities, exercises in the pool are carried out. The third phase of rehabilitation means long-term or lifelong maintenance of the patient. Patients are additionally motivated to improve their lifestyle, stop smoking, eat healthier, and perform regular physical activity.
