Abstract I danas, nakon više od trideset godina od otkrića virusa ljudske imunodeficijencije (HIV) kontrola epidemije infekcije tim uzročnikom predstavlja jedan od najvažnijih javnozdravstvenih prioriteta diljem svijeta. Za infekciju HIV-om karakterističan je olakotan prijenos uzročnika, spolnim putem, krvlju, transplantacijom zaraženih organa i s majke na dijete. Većina infekcija se dogodi nakon spolnog izlaganja, homoseksualnom ili heteroseksualnom transmisijom, pa se prije svega infekcija HIV-om ili HIV-bolest smatra spolno prenosivom bolešću. Nakon ulaska HIV-a u organizam domaćina započinje višegodišnje slabljenje imunološkog sustava osobe uz razvoj brojnih kliničkih manifestacija, od asimptomatske zaraze HIV-om sve do posljednjeg stadija HIV-bolesti koji nazivamo AIDS-om, a koji je praćen oportunističkim infekcijama i nastankom tumora. Specifičnost ovog uzročnika jest nemogućnost njegova uništenja medicinskim tretmanom i izlječenja bolesnika, što posljedično dovodi do velikog broja smrtnih ishoda.
Uz već dovoljno otežan život s HIV-om, oboljele osobe od ove bolesti nerijetko su na meti stigmatizirajućih stavova populacije i visoke razine diskriminacije koja je vezana uz sve aspekte njihova života, pa čak i uz školovanje, zapošljavanje i zdravstvenu zaštitu. Stigmatizacija ne zaobilazi oboljele ni unutar zdravstvenog sustava, a takvo stanje osobito negativno djeluje na mjere prevencije infekcije i dobivanje adekvatne zdravstvene skrbi koja je ključna za postizanje optimalne kvalitete života ovih bolesnika. S obzirom da su medicinske sestre/tehničari zdravstveni djelatnici koji su ponajviše uz bolesnika, prisutstvo stigme u ove skupine zdravstvenih djelatnika je nedopustivo jer u znatnoj mjeri negativno utječe na kvalitetu pružene zdravstvene njege.
Sukladno tome, u svrhu izrade ovog rada u razdoblju od 6. do 13. travnja 2020. godine provedeno je istraživanje online upitnikom na temu stigmatizacije oboljelih od HIV/AIDS-a. Cilj istraživanja bio je odrediti postojanje i učestalost stigmatizirajućih stavova medicinskih sestara/tehničara prema HIV-pozitivnim osobama i osobama oboljelima od AIDS-a. Također, nastojao se dobiti uvid u iskustvo i znanje sudionika. U istraživanju je sudjelovalo ukupno 757 sudionika, od čega 94,3% medicinskih sestara i 5,7% medicinskih tehničara. Dobiveni rezultati pokazuju kako u 81,2% sudionika ovog istraživanja prevladava pozitivan stav prema oboljelim osobama, ali preostalih 18,8% sudionika unatoč napretku medicine i obrazovanja još uvijek ima velik broj negativnih stavova, što upućuje na to kako je stigma i dalje prisutna prema ovoj skupini bolesnika unutar našeg zdravstvenog sustava te i dalje zahtijeva učinkovite strategije za njezino smanjenje.
Abstract (english) Even today, after more than thirty years since the discovery of the human immunodeficiency virus (HIV), controling the outbreak of the infection with this agent is one of the most important global public health prioritie. HIV infection is characterized by the extenuating transmission via sexual pathways, blood, transplantation of the infected organs, and transmission mother to child. Most infections occur after sexual exposure, homosexual or heterosexual transmission, which is why HIV infection is primarily considered a sexually transmitted disease. After HIV enters the host organism, it starts off several years of weakening of the person’s immune system with the development of numerous clinical events – from asymptomatic HIV infection to the last stage of HIV disease, which we call AIDS, accompanied by opportunistic infections and tumour formation. The specificity of this agent is the inability to tackle it with medical treatment, resulting in a large number of deaths.
In addition to a sufficiently difficult life with HIV, diseased individuals are often targeted by stigmatising attitudes of the population and a high level of discrimination related to all aspect of their lives – including education, employment and health care. Stigmatisation does not bypass the sick even within the health care system, and such situation has a particularly negative effect on measures for prevention of infection and obtaining adequate health care which is crucial for achieving the optimal quality of life of these patients. Since nurses/technicians are health care professionals, the presence of stigma in these groups of health care professionals is detrimental because it significantly affects the quality of the provided medical care.
Accordingly, for the purposes of this thesis, in the period between 6 and 13 April, 2020, an online questionnaire was carried out exploring stigmatization of patients with HIV/AIDS. The aim of the research was to determine the existence and frequency of stigmatising attitudes of nurses/technicians towards HIV positive persons and persons with AIDS. Also, this thesis tried to get an adequate insight into the participants’ experience and knowledge. A total of 757 participants participated in the survey, of which 94.3% were nurses and 5.7% were medical technicians. The results show than 81.2% of participans in this survey have a positive attitude towards a person living with HIV/AIDS, but the remaining 18.8% of participants (despite the progress of medicine and education) still have a large number of negative attitudes, suggesting that the stigma is still present towards this group of patients within our health care system and that this problem still necessitates effective strategies.
