Abstract Rak pluća je glavni uzrok smrti povezan s onkološkom bolešću širom svijeta te je postao značajni javno-zdravstveni problem. Zbog postavljanja dijagnoze u kasnijem stadiju i pojavom brojnih komplikacija, ishodi liječenja su skromni, a bolesnici se suočavaju s brojnim psihosocijalnim problemima. Histološka podjela karcinoma pluća dijeli se na karcinom pluća malih stanica (SCLC) i karcinom pluća ne-malih stanica (NSCLC).
Karcinom pluća utječe na kvalitetu života pojedinca i njegove obitelji, a pravilna procjena kvalitete života dovodi do poboljšanja ukupne kvalitete života osobito u bolesnika s terminalnom bolešću. Napredak bolesti, ozbiljnost simptoma i nuspojava smanjuju kvalitetu života kod bolesnika. Kvaliteta života u bolesnika s karcinomom pluća niža je nego u zdrave populacije i kod bolesnika koji pate od drugih malignih oboljenja.
Cilj rada usmjeren je prikazu teškoća s kojima se suočavaju bolesnici s karcinomom pluća te procjeni faktora koji mogu biti povezani s procjenom kvalitete njihovog života. Spomenuti faktori odnose se na obiteljske uvjete, doživljaj podrške, komplikacije bolesti, mogućnosti sudjelovanja u svakodnevnim aktivnostima, uživanje i zabava.
Kao instrument istraživanja upotrijebljen je anketni upitnik koji je sadržavao demografske podatke, a za samoprocjenu kvalitete života upotrijebljena je skala funkcionalne procjene kvalitete života kod bolesnika s malignom bolesti, The Functional Assessment of Cancer Therapy-General (FACT-G).
U svrsi izrade završnog rada proveli smo upitnik u razdoblju od 30.rujna 2019. do 30.travnja 2020. godine putem anonimnih anketnih upitnika. Za istraživanje koristio se posebno strukturirani anketni upitnik „Kvaliteta života oboljelih od karcinoma pluća“ u Općoj bolnici Varaždin, Službi za plućne bolesti i TBC Klenovnik. U istraživanju je sudjelovalo 60 sudionika, od kojih je kronološka dob najmlađeg 42, a najstarijeg 80 godina, svi su liječeni na odjelu pulmologije Službe Klenovnik gdje su primali kemoterapiju putem dnevne bolnice. Od ukupnog broja sudionika njih 71,7% muškog spola i 28,3 % ženskog spola. Najviše sudionika ima prosječna primanja od 3000 do 5000 kn, njih 48%. Prosječna vrijednost ukazuje da ispitanici donekle osjećaju gubitak energije, a mučnine prilično i jako puno osjeća nešto manje od 20% sudionika, a većina ih nema. Zbog zdravstvenog stanja sveukupno 45% sudionika prilično i jako teško ispunjava očekivanja obitelji. Manjem broju sudionika smetaju nuspojave terapije, a većina sudionika nije zbog bolesti prisiljena vrijeme provoditi u krevetu. Podršku okoline u jednakoj mjeri doživljavaju ispitanici muškog i ženskog spola.
Abstract (english) Lung cancer is the leading cause of death linked with oncological diseases worldwide and has become a significant public health problem. Due to diagnosis given in later stages, as well as the occurrence of many complications, the treatment outcomes are poor and patients are faced with many psychosocial problems. Histological classification of lung cancer distinguishes small cell lung carcinoma (SCLC) and non-small cell lung carcinoma (NSCLC).
Lung cancer affects the life quality of an individual and the whole family. A proper life quality assessment can lead to the improvement of overall life quality, especially in the terminally ill patients. Disease progression, severity of symptoms and side effects decrease patients’ life quality. Lung cancer patients’ life quality is lower than in healthy population or in patients suffering from other malignant diseases.
The aim of the final paper is focused on presenting the difficulties that the lung cancer patients are faced with and the study of factors which may be connected with patients’ life quality assessment. Those factors are related to family conditions, experiencing the support, illness complications, chances for participating in everyday activities, enjoyment and fun.
As an instrument of research, the questionnaire with demographic data was used, while for self-evaluation of life quality in malignant disease patients the functional assessment scale was used. The scale is called The Functional Assessment of Cancer Therapy-General (FACT-G).
For the purpose of making the final paper we have conducted a survey in the period between 30th September 2019 and 30th April 2020 by using anonymous questionnaire. A specially structured questionnaire called “Life quality of lung cancer patients” was used for the research in The General Hospital Varaždin, Pulmonary Disease Service and Tuberculosis Hospital Centre Klenovnik. In the research participated 60 patients, the youngest of which was 42, and the oldest was 80, all treated at the pulmonology department of Klenovnik Service where they have been receiving chemotherapy in day care hospital. Of all participants, 71,7% were male and 28,3% were female gender. Most participants have average incomes between 3000 and 5000 kn, 48 % of them. In average, participants feel the loss of energy to some extent, and nausea is felt quite a lot and very much in slightly less than 20 % of participants, while the majority doesn’t feel it. Due to their health conditions 45 % of participants find it very difficult to meet their families’ expectations. Less participants feel troubled by the side effects of treatments, while the majority of participants isn’t forced to spend time in bed due to illness. Both male and female participants feel to receive equal social support.
