Abstract Celijakija je autoimuna bolest, poremećaj koji obuhvaća uroĎeni i stečeni imunitet kod genetski predisponiranih ljudi. Nastaje uslijed ingestije glutena koji zbog imunološke reakcije uzrokuje promjene u strukturi sluznice tankog crijeva što u konačnici dovodi do malapsorpcije nutrijenata i pojave simptoma celijakije. Najčešće se pojavljuje u dobi malog djeteta ili kasne dojenačke dobi zbog uvrštavanja ţitarica bogatih glutenom u prehranu, no valja istaknuti da je pojava celijakije moguća u svakoj ţivotnoj dobi. Simptomi koji prate celijakiju i prema kojima se moţe posumnjati na istu su abdominalna bol i grčevi, proljevaste stolice masne konzistencije ili pak pojava opstipacije, anemije, rahitisa, sporiji rast i razvoj kod djece i adolescenata, gubitak na tjelesnoj teţini, umor i slabost, a metoda izbora prilikom dijagnosticiranja celijakije je biopsija sluznice tankog crijeva. Komplikacije celijakije koje nastaju uslijed ne pridrţavanja bezglutenske dijete su hiposplenizam, refraktorna celijakija, crijevni limfom, adenokarcinom tankog crijeva, ulcerativni jejunoileitis i celijakična kriza. To su ozbiljna i po ţivot opasna stanja. Prema današnjim saznanjima, smatra se da je celijakija kronična neizlječiva bolest, no uz pomoću striktne bezglutenske dijete moguće je drţati simptome pod kontrolom i u potpunosti otkloniti negativne posljedice bolesti. Kako bi pacijent što lakše i efikasnije savladao potrebne vještine i znanje vezane uz ţivot sa celijakijom te provedbom bezglutenske dijete, od velike vaţnosti je edukacija pacijenta i njegove obitelji od strane medicinskih sestara/tehničara i ostalih članova multidisciplinarnog tima. Iako je celijakija definirana kao kronična neizlječiva bolest, u vrlo rijetkim slučajevima zabiljeţena je tolerancija na gluten kod osoba kojima je ponovno uveden gluten u prehranu nakon dugogodišnje provedbe bezglutenske prehrane i rezultata biopsije koji upućuju na potpunu obnovu sluznice tankog crijeva kao što je prikazano u prikazu slučaja ovog rada.
Abstract (english) Celiac disease is an autoimmune disease, an disorder that includes both innate and acquired immunity in genetically predisposed people. It appears with ingestion of gluten that causes immune reactions that causes changes in the stucture of small bowel mucosis that finally leads to the malabsorption of nutrients and the onset of the celiac disease symptoms. It most often occurs in the age of a small child or late infancy due to the inclusion of gluten-rich cereals in the diet, but there is also possibility that it can occur in every human age. Symptoms that come with celiac disease are abdominal pain and cramps, diarrheal stools of oily consistency or constipation, anemia, rickets, slower growth in children and adolescents, weight loss, fatigue or weakness. The method of choice when diagnosing celiac disease is a biopsy of the small intestinal mucosa. Complications of celiac disease resulting from non-adherence to a gluten-free diet are hyposplenism, refractory celiac disease, intestinal lymphoma, small bowel adenocarcinoma, ulcerative jejunoileitis, and celiac crisis. These are serious and life-threatening conditions. According to today's findings, celiac disease is considered a chronic incurable disease, but with the help of a strict gluten-free diet it is possible to keep the symptoms under control and completely eliminate the negative consequences of the disease. In order for the patient to easily and effectively master the necessary skills and knowledge related to living with celiac disease and implementing a gluten-free diet, it is important to educate the patient and his family by nurses and other members of the multidisciplinary team. Although celiac disease is defined as a chronic incurable disease, in very rare cases gluten tolerance has been reported in people who have been reintroduced gluten into their diet after many years of gluten-free diet and biopsy results suggesting complete regeneration of the small intestinal mucosa as shown in the case report of this paper.
