Abstract
Rotatorna menšeta, mišićno-tetivna je ovojnica koja ramenu pruža stabilnost i snagu pomoću svoja 4 mišića od kojih je sačinjena: m. supraspinatus, m. infraspinatus, m. subscapularis i m. teres minor. Glavne zadaće koje ona ima su da omogući pokretljivost nadlaktične kosti u odnosu na lopaticu te da stabilizira glavu nadlaktične kosti u glenoidnoj šupljini. Upravo su oštećenja tetiva rotatorne manšete jedan od najčešćih uzroka pojave boli i ostalih tegoba u ramenu. Oštećenje i jedne tetive mišića rotatorne manšete uvelike smanjuje funkciju ramena i ruke zahvaćene strane. U kliničkoj slici najčešći simptom koji se javlja je bol, lokalizirana s vanjske strane ramena i najčešće kod poslova i aktivnosti koje zahtijevaju učestalo dizanje ili držanje ruke iznad razine ramena. Najčešće se rupture rotatorne manšete javljaju kod osoba srednje i starije životne dobi kao posljedica degenerativnih procesa ali i kod mlađih koji se bave aktivnostima u kojima se učestalo diže ruka iznad razine ramena uz češće pojavljivanje kod osoba ženskog spola. Glavne dijagnostičke metode kojima dokazujemo prisutnost rupture su anamneza, klinički pregled te ultrazvuk i magnetska rezonancija. Osobama starije životne dobi kao metodu liječenja preporuča se konzervativno liječenje što uključuje mirovanje, uzimanje nesteroidnih protuupalnih lijekova te fizikalna terapija dok se mlađim pacijentima preporuča operacijsko liječenje uz kasniju rehabilitaciju za što brži povratak svakodnevnim aktivnostima te sportovima ako se njima bave. Operacijski zahvati dijele se na otvorene i artroskopske. Svaki od njih ima svoje prednosti i nedostatke no rezultati su funkcionalno jednaki. Nakon operacije slijedi fizioterapijska procjena tijekom koje se kroz subjektivni i objektivni pregled analizira stanje pacijenta i izrađuje plan terapija. Nakon operacije 3-4 tjedna pacijent nosi abdukcijsku ortozu, a s terapijama započinje odmah drugi dan nakon operacije kako bi prvo kroz pasivne vježbe postigli određeni opseg pokreta u prvih 6 – 9 tjedana, dok se nakon 14 tjedana fokusiramo na jačanje mišića rotatorne manšete i vraćanja samostalnosti u provođenju aktivnosti svakodnevnog života.
Abstract (english)
Rotator cuff is a muscular-tendon sheath that provides stability and strenght to the shoulder with the help of its 4 muscles: m.supraspiuatus, m. iufraspinatus, m. subscapularis and m. teres minor. The main tasks that rotator cuff have are to enable mobility of humerus in relation to scapula and to stabilize the head of the humerus in the glenoid cavity. Damage to the tendon of the rotator cuff is one of the main couses of the appearance of pain and the other problem sin the shoulder. Damage to only one rotator cuff tendon greatly reduces the function of the shoulder and most often occurs in jobs and activities that require frequent fasing or holding your hand above shoulder level. Most often, rotator cuff ruptures occur in people of middle and older age as a result of degenerative processes, but also in younger people angaged in activities in which the hand rises frequently above shoulder level with a more frequent appearance in people of the female sex. The main diagnostic methods by which we prove the presence of rupture are medical history, clinical examination, ultrasound and magnetic resonance. Elderly people are advised to recieve conservative treatment that includes rest, taking non-steroidal anti-inflammatory drugs and physical therapy, while younger people are recommended to udergo surgery with later rehabilitation for the quickest possible return to daily activities and sports if they are engaged in. Surgery procedures are divided into open and artroscopic. Each of them has its advantages and disadvantages but the results are uniformly the same. The surgery is followed by a physiotherapy assessment during which, through subjective and objective examination, the patient's condition is analyzed and a therapy plan is drawn up. 3-4 weeks after surgery, the patient carries abductive orthosis, and with physical therapy begins immediately the second day after the operation. The first 6-9 weeks, through passive range of motion aim to achieve a certain range of motion while after 14 weeks we fasing on strengthening the muscles of the rotator cuff as well as restoring independence in the coonduct of activities of everyday life.
