Abstract
Dijabetes mellitus je kronična bolest uzrokovana poremećajem izlučivanja inzulina te se manifestira povećanjem koncentracije glukoze u krvi. Razlikuju se dva tipa dijabetesa, tip I i tip II. Kod dijabetesa tipa I dolazi do propadanja beta stanica gušterače pomoću kojih se proizvodi inzulin te dolazi do nedostatka inzulina. Dijabetes tip II uzrokovan je neosjetljivošću stanica na inzulin ili manjkavom funkcijom gušterače. Neosjetljivost stanica na inzulin (tzv. inzulinska rezistencija) definira se kao neadekvatan odgovor stanica na inzulin koji je prethodno stvoren u gušterači. Stanična nemogućnost korištenja inzulina potiče gušteraču na pojačano stvaranje dodatnog inzulina. Posljedično, u krvotoku dolazi do povećanja inzulinske rezistencije što dovodi do iscrpljenja gušterače te visoke razine šećera u krvi koji ne može ući u stanice. Dijabetes tip I je češći oblik kod djece, no u posljednje vrijeme, zbog sjedilačkog načina života i nezdrave prehrane, sve više djece obolijeva od dijabetesa mellitusa tipa II. Kada nastupi dijabetes, izlječenje nije moguće, već je potrebno prilagoditi se i stvoriti novi način života, kako kod djece, tako i kod odraslih. Potrebno je mijenjati životne navike. Uz prihvaćanje prijedloga i jelovnika stručnjakâ s područja dijetetike, vrlo je važno uvesti više fizičke aktivnosti, kontrolirati tjelesnu težinu, vršiti česte samopreglede te njegu stopala kako ne bi došlo do razvoja komplikacija šećerne bolesti. Tjelesna aktivnost je izrazito važna za djecu s dijabetesom te je preporučeno svakodnevno vježbanje. Tjelovježbom se smanjuje razina šećera u krvi te se u nekim slučajevima smanjuje doza inzulina (uz redovito vježbanje). Također, tjelesna aktivnost ima utjecaj i na količinu i izbor hrane koju dijete unosi. Npr. prije vježbanja djetetu je dozvoljeno dati komadić hrane bogat ugljikohidratima (npr. kruh, čokolada) što u suprotnome nije preporučljivo. Uloga magistre sestrinstva jest edukacija o prevenciji dijabetesa mellitusa te pružanje poticaja za uvođenje javnozdravstvenih projekata u vrtiće i škole.
Abstract (english)
Diabetes mellitus is a chronic disease caused by a disorder of insulin secretion and is manifested by an increase in blood glucose levels. There are two types of diabetes, type I and tip II. In type I diabetes, the pancreatic beta cells that produce insulin are depleted and insulin is deficient. Type II diabetes is caused by cell insensitivity to insulin or deficient pancreatic function. Cell insensitivity to insulin (so-called insulin resistance) is defined as the inability of a cell to produce insulin previously produced in the pancreas. Cellular inability to use insulin stimulates the pancreas to increase the production of extra insulin. Consequently, there is an increase in insulin resistance in the bloodstream leading to pancreatic depletion and high blood sugar levels that can enter the cells. Type I diabetes is a more common form in children, but lately, due to a sedentary lifestyle and unhealthy diet, more and more children are suffering from type II diabetes mellitus. When diabetes occurs, a cure is not possible but it is necessary to adapt and create a new way of life, both in children and adults. It is also necessary to change life habits. In addition to accepting the suggestions and menus of experts in the field of dietetics, more physical activities, controlled body weight, frequent self-examinations and foot care are often introduced to prevent the development of complications of diabetes. Physical activity is extremely important for children with diabetes and daily exercise is recommended. Exercise lowers blood sugar and in some cases reduces the dose of insulin (with regular exercise). Also, physical activity has an impact on the amount and choice of food a child ingests. E.g. before exercising the child can have a piece of food rich in carbohydrates (e.g. bread, chocolate) which is otherwise not recommended. The role of the Master of Nursing is to educate about the prevention of diabetes mellitus and to provide incentives for the introduction of public health projects in kindergartens and schools.
