Abstract
Infarkt miokarda je nekroza tkiva miokarda nastao kao posljedica relativnog ili apsolutnog nedostatka opskrbe miokarda krvlju.[1] Kardiovaskularne bolesti su značajni javnozdravstveni problem diljem svijeta. Glavni su uzrok smrti u razvijenim zemljama svijeta, a u manje razvijenim dijelovima svijeta smrtnost je u porastu i prestiže stope smrtnosti od infektivnih bolesti. Bolesnici sa infarktom miokarda liječe se u jedinicama intenzivne koronarne skrbi. Planiranje zdravstvene njege u intenzivnoj... More koronarnoj jedinici obuhvaća utvrđivanje prioriteta, definiranje ciljeva, planiranje intervencija te izradu plana zdravstvene njege.[11] U jedinici intenzivne koronarne skrbi rade medicinske sestre koje su posebno educirane za rad u njoj. Bolesnici imaju dvadesetčetverosatnu skrb. Intervencije medicinske sestre u radu s bolesnikom u akutnoj fazi infarkta miokarda usmjerene su na vitalno zbrinjavanje bolesnika, asistiranje i sudjelovanje u kradiopulmonalnoj reanimaciji, te psihičkoj i fizičkoj pripremi bolesnika za invazivnu koronarnu intervenciju. Tako je i SZO propisala i uredila oblike trajnog usavršavanja medicinskih sestara u području zdravstvene njege bolesnika u jedinicama intenzivnog liječenja. Preporuka međunarodnih sestrinskih udruga (Međunarodna udruga medicinskih sestara-ICN, 1987, 1992; Europski komitet za sestrinstvo-European Commisions Advisory Committee on Nursing 1994, Europian Directives 89/48 CEE i 92/51 /eec) jest da se medicinske sestre u svojoj edukaciji, a poslije i djelatnosti, usmjeravaju u uža specijalistička područja. Time dobivaju detaljnije teorijske i praktične spoznaje, a sustav zdravstvene njege postaje znatno kvalitetniji. Ciljevi novih programa teže proširivanju ne samo općih nego posebno specifičnih znanja i vještina u području zdravstvene njege i stjecanja kritičkoga načina razmišljanja.[7] Less
Abstract (english)
Myocardial infarction is a myocardial tissue necrosis which occurs as the result of relative or absolute deficiency of of myocardial blood flow.[1] Cardiovascular diseases are a major health problem worldwide. They are a major cause of death int he developed countries, and in developing countries the mortality rate is increasing, and currently exceeds the mortality rate oft he infectious diseases. The patients with myocardial infarction are being treated in the coronary intensive care units.... More The planning of health care in the coronary Intensive Care Unit includes determining priorities, defining of goals, planning of interventions and preparation of the health care plan.[11] The coronary intensive care unit employs nurses who are additionally educated for such work. The patients have 24-hour care. The interventions of a nurse as regards care of the patient during the acute phase of the myocardial infarction are directed towards the vital care for the patient, assistance and participation in the cardiopulmonary reanimation, and psychological and physical preparation of the patient for the invasive coronary intervention. Thus the WHO also stipulates and regulates the form of permanent education of nurses in the area of health care of the patients in the intensive care units. The recommendation of the International Council of Nurses (International Council of Nurses - ICN, 1987, 1992; European Commission Advisory Committee on Nursing 1994, European Directives 89/48 CEE and 92/51 /EEC) is that the nurses in their education, and later during their practice, are directed towards narrower specialist areas. Thus they receive more detailed and practical knowledge, and the health care system reaches higher levels of quality. The goals of the new programs are directed towards the widening of not only the general, but also of the specialist knowledge and skills in the area of health care and acquisition of critical thinking.[7] Less
