Abstract Pojam aneurizma Galenove vene povezan je sa skupinom različitih vaskularnih
anomalija, s jednom zajedničkom razlikom, a to je dilatacija Galenove vene.
Predstavlja rijetku vaskularnu anomaliju, čija učestalost još nije poznata. Iako su
rijetke, ove su anomalije od posebnog interesa za interventne radiologe, jer je
potencijalno endovaskularno liječenje koje se pokazalo učinkovitim, a često i jedinim
sigurnim terapijskim modalitetom, promijenilo općenito loš ishod u novorođenčadi i
dojenčadi (visok rizik od neuroloških poremećaja, zatajenja srca, visoku stopu
smrtnosti do 90%) i malo su u današnje vrijeme poboljšali njihove šanse za
preživljavanje. Pacijenti s aneurizmom Galenove vene najčešće imaju srčane i
neurološke komplikacije, a klinička slika ovisi o dobi prezentacije. Novorođenčad ima
tendenciju zatajenja srca visokog srčanog izbačaja, plućne hipertenzije i, u težim
slučajevima, zatajenja višeorganskih sustava. Dojenčad obično ima hidrocefalus,
konvulzije ili neurokognitivno kašnjenje. Starija djeca i odrasli obično imaju
glavobolje ili intrakranijalno krvarenje.Većina malformacija se dijagnosticira u
neonatalnom razdoblju, dok se prenatalna dijagnoza obično postavlja tijekom trećeg
tromjesečja trudnoće ultrazvukom prema vizualizaciji anehogene ili hipoehogene
okrugle ili ovoidne strukture s pravilnim rubovima smještene u središnjoj liniji
stražnjeg dijela treće klijetke. Razvoj endovaskularnih tehnika uvelike je poboljšao
lošu prognozu pacijenata. Sveukupni cilj endovaskularnog liječenja je vratiti
hemodinamsku ravnotežu i postići fiziološko, a ne anatomsko, izlječenje koje dovodi
do povoljnih neuroloških i razvojnih ishoda. Ključno je da medicinske sestre/tehničari
posjeduju sveobuhvatno znanje o različitim tehnikama koje se koriste za pristup
femoralnoj arteriji, mogućim komplikacijama i zdravstvenoj njezi koja je potrebna za
učinkovito upravljanje tim komplikacijama.
Abstract (english) The term aneurysm of the vein of Galen is associated with a group of different vascular
anomalies, with one common difference, which is the dilatation of the vein of Galen.
It represents a rare vascular anomaly, the frequency of which is not yet known.
Although rare, these anomalies are of special interest to interventional radiologists,
because potentially endovascular treatment, which has been shown to be effective and
often the only safe therapeutic modality, has changed the generally poor outcome in
newborns and infants (high risk of neurological disorders, heart failure, high mortality
rate of up to 90%) and have improved their chances of survival little nowadays.
Patients with an aneurysm of the vein of Galen most often have cardiac and
neurological complications, and the clinical picture depends on the age of presentation.
Newborns tend to have high cardiac output heart failure, pulmonary hypertension and,
in more severe cases, multi-organ system failure. Infants usually have hydrocephalus,
convulsions, or neurocognitive delay. Older children and adults usually have
headaches or intracranial bleeding. Most malformations are diagnosed in the neonatal
period, while prenatal diagnosis is usually made during the third trimester of
pregnancy by ultrasound visualization of an anechoic or hypoechoic round or ovoid
structure with regular borders located in the midline of the posterior third ventricle.
.The development of endovascular techniques has greatly improved the poor prognosis
of patients. The overall goal of endovascular treatment is to restore hemodynamic
balance and achieve physiologic, rather than anatomic, healing that leads to favorable
neurologic and developmental outcomes. It is critical that nurses/technicians have a
comprehensive knowledge of the various techniques used to access the femoral artery,
potential complications, and the nursing care required to effectively manage these
complications.
